Documents required for renewal, replacement
or applying for a first-time Jamaican Passport.
All Jamaican Passport applications must include:
a completed Jamaican Passport Application Form
an original or certified copy of your Jamaican birth certificate.
two colored passport photos (professional quality photos only**).
** Drugstore (Walgreens, CVS) passport photos are usually not accepted.
by the Jamaican Passport Office - PICA.
Additional documents that may be required include:
your actual expired Jamaican Passport
a certified copy of your marriage / divorce certificate.
a certified copy of a death certificate.
a valid state issued photo ID if applying for a lost / stolen Jamaican Passport.
a police complaint report for lost / stolen Jamaican Passport (if available).
a lost / stolen Jamaican Passport Application Form (provided with our service).
if applying for a lost / stolen Jamaican Passport.
valid proof of parent / guardianship for a minor child's Jamaican Passport.
a certificate of adoption.
a certificate of Jamaican Naturalization.
a certificate of Jamaican Citizenship.
